Current guidelines for use of intravenous thrombolysis for acute stroke generally recommend initiation within 4.5 hours of symptom onset. This multicenter, placebo controlled RCT;

it is important to note that even delayed beyond 4 to 6 hours TLT in stroke,is associated with an increased chances of good neurologic outcomes; even if, there is also an associated increased risk of cerebral hemorrhage!!
